LOG FILES
Colegio San Agustin – Bacolod follows a standard procedure of using log files. These files log visitors when they visit websites. All hosting companies do this and a part of hosting services’ analytics. This Privacy Statement describes how we at Colegio San Agustin – Bacolod collect and process personal data and the school’s legal obligations generally in relation to the provision of education. Processing is the term used to describe various acts including – the collection, recording, organization, structuring, storage, alteration, use of, retrieval, disclosure or transmission of information.
WHY WE COLLECT PERSONAL INFORMATION?
Colegio San Agustin – Bacolod collects your personal information to ensure that we provide appropriate support and to ensure delivery of the services needed and requested. These data help us get our services right to meet individual needs.
WHAT PERSONAL DATA WE COLLECT?
Personal data refers to any information you provided in the different forms during Admission/Enrollment. It also includes supporting documents (birth certificate, report card, good moral character, ID card, photos, transfer credentials and NCAE Exam result) requested as part of admission.
Application for Admission (Pre-Enrollment)
We collect personal information and sensitive personal information like name, course applied, year level, gender, civil status, religion, nationality, birthday, place of birth, contact number, home address, email address, height (optional), weight (optional) and last school attended.
During Enrollment
When the student decided to enroll, the additional information are needed: family background like father’s name, mother’s name and their occupations.
Application for Student’s ID
We collect information like your student’s ID number, name, year level, address, birthday, blood type, parent/guardian’s name and parent/guardian’s mobile number (in case of emergency).
HOW WE USE YOUR INFORMATION?
As an educational institution, we use this information for the following purposes.
- To process in taking your entrance examination
- To assess applications for admission
- To process your application on enrolment
- To be used during applicant’s interview
- To process your payments and other fees
- For administrative purposes
- For processing scholarships, financial assistance and other privileges
- For academic deliberations and evaluation of student’s performance
- To process your requests such as Transcript of Records, Form 9, Certifications, etc.
- For research and statistical purposes
- For compliance with legal obligations
- For conducting surveys
- To deliver facilities and provide different services (medical/dental services, Wi-Fi access, library services, religious services, IMC, and guidance services)
- For student’s recording of coming in and out of campus (using RFID in turnstile and Library)
- To communicate effectively by post email and phone
- For maintaining directories and manage School’s Information System
- To create an account for computer laboratories
- To create an account in checking of grades online
- For communicating official school announcements
- To create an account for Learning Management System (LMS) – see CSA-B LMS Acceptable Use Policy
- To protect your health and safety in case of emergency
- For investigating of incidents relating to student, faculty, or employee discipline
- To maintain security inside the campus (using CCTV cameras ( see CCTV Policy))
- For annual physical examination
- For telemedicine or virtual check-up (voluntarily)
- To process your information for the purpose of Immersion/On-the-Job Training
- To fulfil your contractual obligations to the School (See MOA in Enrollment Form)
HOW WE SHARE AND DISCLOSE YOUR PERSONAL INFORMATION?
Colegio San Agustin – Bacolod shares information in the pursuit of its legitimate interests and responsibilities as an educational institution, in accordance with regulations prescribed by law. We may share or disclose your information with your consent with the following:
- Submission of report to the different government agencies like CHED, UniFast, PEAC, and DepEd.
- We share your information to PRC, Bureau of Immigration, DFA and Civil Service Commission only when required.
- For accreditation, we share your information to accrediting agency like PAASCU only when required.
- We post your photos and names on our website/official Facebook page when you passed the Board Examinations.
- We share your names through class list and grade sheet to your adviser or teacher.
- In case of emergency, we disclose information to the clinic, hospital and insurance company.
- We post your name when you are a dean’s list honors, graduating students or you have lack of requirements (names only) on the bulletin board.
- We share your information to the donors or benefactors and other forms of scholarships and assistance (for those who applied only).
- We share your information to some cultural, sports and athletic competitions like NOPSSCEA (for athletes/participants only).
- We share result of surveys to the different Offices (required to use the result of surveys). We do not include your name. Mostly, we share statistics only.
- We share your information for pre-employment and background checking (with consent form attached in email).
- We share information to the systems programmer/systems provider if there’s an error, for testing, or updating in the School’s information system.
- With your consent, we use and share your photos/videos in promoting the School through Marketing Office, in printed materials and posted in website and Facebook (Student Media and Release Form).
14. We share your information for reporting in National Privacy Commission (NPC) for data breaches and other legal obligations.
HOW WE PROTECT AND RETAIN PERSONAL INFORMATION?

We retain your personal information as long as it is needed unless there is a retention policy required by the law.

You can request a copy of the personal data we hold about you, including any data you have provided to us. You may also request to modify, correct or update the personal information or sensitive personal information that you provided to us. You can also request that we erase any personal data we hold about you. This does not include any data we are obliged to keep for administrative, legal, or security purposes.
Under RA 10173 or the Data Privacy Act of 2012, you have the right to be informed, to object, to access, to rectification, to erasure, to damages, to data portability and to file a complaint.

We may need to modify and amend this privacy statement from time to time to reflect our current privacy practices. Any amendments or modifications to this Privacy Statement may be effected by CSA-B through posting changes on website, written notice or email sent to you.

WHAT PERSONAL DATA WE COLLECT?
We obtain basic personal information from you in our Visitor’s Log Sheet, which includes but is not limited to date and time, name, purpose of visit, person to see, department, and signature. You are required to present any valid ID for verification purposes. You are also required to wear the visitor’s pass throughout your visit (See Access to School Premises Policy). Video footage is also being recorded on our CCTV system installed in various places inside the campus (See CCTV Policy).
WHY WE COLLECT PERSONAL INFORMATION?
Primarily, we collect personal information for safety and security purposes only.
HOW WE USE, PROTECT AND RETAIN PERSONAL INFORMATION?
Your personal data is maintained in a secure manner. Only authorized security personnel have access to them. We will only use the personal information we collect for the purpose for which we collected it, namely to record your details in order to provide you with the appropriate identification while inside the campus.
We will only keep your personal data for a period of one (1) month from the date of collection, unless required by law to retain them for a longer period. CCTV recordings are stored for seven (7) to fifteen (15) days before being automatically deleted (See CCTV Policy).
We do not transfer or share your personal data with other persons or organizations.

PERSONAL DATA COLLECTED: Name, Sex, Age, Address, Health status (checklist) and Temperature.
For students having face-to-face classes, additional data to be collected: courses and sections.
PURPOSE: For health and safety purposes (Source: DTI and DOLE Interim Guidelines on Workplace Prevention and Control of COVID-19).
DISCLOSURE: Only the school safety officer, clinic staff and authorized personnel have the access of your data. We will not give, sell or rent this information to anyone. We only collect what is necessary. Disclose only to the proper authority like DOH and IATF ( in case of COVID-19 contact tracing). (see NPC PHE BULLETIN No. 3)
RETENTION AND DISPOSAL: We will only keep your personal data for a period of one (1) month from the date of collection, unless required by law to retain them for a longer period.
